I.-A health care organisation is eligible for inclusion on the list of local hospitals provided for in article L. 6111-3-1 if it meets all the conditions mentioned in II. A site belonging to a health facility that does not meet the conditions mentioned in II is eligible for inclusion on the list if this site itself meets these conditions.

II.-The conditions of eligibility for inclusion of a health care organisation or an identified site of a health care organisation on the list of local hospitals are as follows:

1° The health care organisation, or the site, cooperates with the health care players in its territory providing primary care as defined in article L. 1411-11 under the conditions set out in article R. 6111-25. This cooperation may, in particular, take the form of private practice or salaried employment, within the local hospital, of healthcare professionals monitoring patients and coordinating healthcare pathways within the outpatient services;

2° The establishment, or the site, carries out a medical activity authorised by the regional health agency in application of the provisions of article L. 6122-1, and is not authorised to carry out surgery or gynaecology-obstetrics in application of the same provisions;

3° The establishment, or the site, offers, as part of its medical activity, a range of care that is not solely intended for the treatment of a specific pathology or a particular population category;

4° The establishment or site offers consultations in several specialities, carried out by doctors practising either on a self-employed basis or as employees or public servants in the establishment or site or in partner health establishments;

It has its own imaging and medical biology technical platforms and telehealth equipment, or has access to them under an agreement, taking into account the resources available in the region. This care offer is complementary to the available outpatient offer and, where appropriate, is consistent with the health project of the territorial professional health community in the territory of the establishment or site;

5° The establishment, or the site, carries out the missions mentioned in 1° to 4° of II of Article L. 6111-3-1, in complementarity with the healthcare services available in the territory it serves.
