The "Oncological surgery" modality comprises the following entries:

I.-Category A providing oncology surgery in adults for one or more of the following seven tumour locations, mentioned in the authorisation, and excluding complex surgery mentioned in category B:

1° A1: Visceral and digestive oncology surgery;

2° A2: Thoracic oncology surgery;

3° A3: Otorhinolaryngeal, cervicofacial and maxillofacial oncological surgery, including thyroid cancer surgery;

4° A4 : Urological oncological surgery;

5° A5 : Gynaecological oncological surgery;

6° A6 : Breast oncological surgery;

7° A7 : Undifferentiated oncological surgery. The tumour location(s) in A7 do not concern the locations provided for in 1° to 6°, with the exception of thyroid cancer surgery mentioned in 3°.

II.-Category B providing, in addition to the adult oncology surgery authorised in category A, a referral service as well as complex multivisceral or multidisciplinary surgery or surgery for the recurrence of malignant tumours in adults or oncology surgery in irradiated areas, for one or more of the five tumour locations provided for in 1° to 5° below, the type of which is specified in the authorisation:

1° B1: Complex visceral and digestive oncology surgery, including peritoneal damage.

The specific therapeutic practices mentioned in article L. 6122-7 for B1 are:

a) The referral mission mentioned in article R. 6123-91-2 as well as complex multivisceral or multidisciplinary surgery or surgery to treat recurrence, curative malignant tumours in adults or oncological surgery in irradiated areas;

b) Oncological surgery of the oesophagus or gastro-oesophageal junction;

c) Oncological surgery of the liver;

d) Oncological surgery of the stomach;

e) Oncological surgery of the pancreas;

f) Oncological surgery of the rectum.

At the request of the applicant, authorisation for complex visceral and digestive oncology surgery may be limited to one or more of the specific therapeutic practices mentioned in 1°, including at least that mentioned in a of 1°.

The specific therapeutic practice or practices implemented under B1 are specified in the application for authorisation and mentioned in the authorisation decision;

2° B2: Complex thoracic oncology surgery, including surgery for cancers of the trachea, cancers invading the spine, heart or chest wall;

3° B3: Complex oncological surgery of the otorhinolaryngeal, cervicofacial and maxillofacial spheres, including the practice of excision surgery with complex reconstruction in the same operating time as the excision;

4° B4: Complex urological oncological surgery, including the practice of surgery for cancers with vascular or lombo-aortic involvement;

5° B5: Complex gynaecological oncological surgery, including surgery for cancers with peritoneal involvement.

The specific therapeutic practices mentioned in article L. 6122-7 for category B5 are:

a) The referral mission mentioned in article R. 6123-91-2 as well as complex multivisceral or multidisciplinary surgery or surgery for recurrence, curative of malignant tumours in adults or oncological surgery in irradiated areas, including for cancers with peritoneal involvement;

b) Surgery for ovarian cancers.

Authorisation for complex gynaecological oncological surgery may be limited, at the request of the applicant, to a in section B5.

The specific therapeutic practice(s) implemented in section B5 are specified in the application for authorisation and mentioned in the authorisation decision.

III - Option C providing oncology surgery for children and adolescents under the age of eighteen.
